Rabb Care – Admissions Procedure

At Rabb Care, our admissions process is designed to ensure a smooth, informed, and child-centred transition. Each referral is carefully assessed in collaboration with local authorities and professionals, with personalised planning and preparation to meet the individual needs of every young person joining our service.

1. Initial Acceptance

Referrals are accepted by our Directors and Operations Manager through phone or email contact with our head office. Referrals can be sent directly to:

2. Initial Consultation

An initial consultation takes place via phone, where professionals share key information. This communication may continue through follow-up emails.

Referral Information

The Senior Manager overseeing admissions will request comprehensive written details about the referral, such as a completed referral form from the local authority.

4. Referral Assessment

Once the referral is deemed potentially suitable, the information is shared with the Registered Manager of the relevant home where a placement vacancy exists.

5. Suitability Review

A joint decision is made by the Home Manager and the Senior Manager at head office to determine placement suitability. This review and feedback to the local authority typically occur on the same day the referral is received.

6. Provider’s Response

A detailed Provider’s Response Form is completed by the Senior Manager and sent to the local authority. This outlines how Rabb Care will meet the young person’s individual needs.

7. Placement Confirmation

Confirmation of the placement is then received from the local authority, allowing the transition plan to progress.

8. Rabb Care Team Visit

At this stage, a member of the Rabb Care team will arrange to visit the young person as part of their admission process.

9. Young Person Visit

If appropriate, or instead of a team visit, we will organise a visit for the young person to view the home and meet key staff.

10. Risk and Needs Assessment

The Home Manager and staff complete thorough risk assessments, initial care strategies, and a 24-hour support plan. This process is carried out in collaboration with the social worker and staff from the young person’s current placement to ensure all information is current and accurately assessed.

11. Team Preparation

All relevant details are shared with the care and education team to ensure they are well-informed and fully prepared for the young person’s arrival.

12. Informing Current Residents

Young people already residing at the home are sensitively informed and given appropriate information about the new placement.

13. Personalised Bedroom Preparation

Following the pre-placement meeting, the young person’s bedroom is redecorated in line with their personal preferences. This may include chosen posters, pictures, or themed décor. High-quality furnishings and a Welcome Pack (including a Young Person’s Guide, toiletries, towels, and reading materials) are also provided.

14. Keyworker Assignment

A dedicated keyworker is assigned to the young person, and their Clear Care record is activated to track support and planning.

15. Placement Planning Meeting

A placement planning meeting is then scheduled at the home, ensuring all professionals and carers are aligned on the care and support strategy.
